Wellness Report 3 – MOBILITY

Hi Helen, I want to take just a couple of minutes again to review the results of your recommended care and once again I am so happy with your progress! Let me ask you – Are you feeling good about where you are at physically? (Great!) That is awesome – chiropractic works! You have come a long way since your first visit. I want to remind you again of how far you have come. You had a great deal of pain in the lower back radiating down into the right leg, making it difficult for you to walk and get around. It was naturally keeping you from being able to do any of your daily activities. It was keeping you up at night. I know you are glad to be done with that – and we want to keep it that way!

(Summarize The Progress You Are Seeing) I am glad to say, you have been getting stronger and stronger! I can tell so much difference and your body is responding just as I had hoped. You are stabilized and your muscles are remembering the proper position. You are exactly where I hoped you would be at this point in your care!

(% Improvement) This means it is time to move you to Wellness Care. At the end of your appointment today, Sally will schedule this wellness care, so you know exactly what to do. All you have to do is follow the directions! I am going to put you on a once a month wellness plan because I believe that will keep you in a stabilized condition. Remember, it is impossible for me to anticipate and know when you do something that could cause an injury. But as long as your activities remain normal like these last couple of months, I feel confident that once a month will keep you stabilized.

(Schedule of Care) SO NOW THAT YOU’VE REACHED DESTINATION WELLNESS – you will need to come in once a month for the year… only 12 visits to keep you properly maintained in a wellness condition. But that’s not all! We want to reward you for being such an incredible patient and for following the directions so well by giving you VIP status at our office! On this flyer is a QR code you can scan with the camera on your phone to sign up for our Destination Wellness Rewards Program. Simply signup for the FREE program and add the digital loyalty reward card to your phone’s mobile wallet. Then after each visit you can scan your card at the front desk and every eighth wellness visit will be FREE! Sound good? (Allow them to respond)

Cheer again after signing and take them to the front desk so they can AMS their future wellness visits!)
